Program & Case Management

Program and case management is your team behind the scenes that supports progress in your programme and adapts it to changes and newfound information as it becomes available.

They connect all of your treatment team with daily communications and case notes, monitor all aspects of your schedule and programme, monitors biometric data, supply applicable resources, sources treatment team specialists and further medical history if accessible.

In a programme with as much in it as Goodsky, it needs to be managed and this is the work we do to make sure your programme runs smoothly.

Behavioural Naturopathy

Already underway and budgetted

Your mental health naturopath will assess your situation, lifestyle habits, nutrition (food logs), test and assess neurotransmitter levels through gut bacterial metabolism and assessments, digestive absorption, genetics, stress hormones and much more.

You will be sent specific pathology tests listed below based on presenting conditions that will form part of your treatment foundation. We can also do a variety of other assessments to uncover causation, which then leads to treatment.

Appointment: Initial consultation + pathology consultation to order testing which can be completed where you are.

Extensive Testing

Already underway and budgetted

The following pathology panel is a Goodsky test panel that gives us excellent data across the board. Once an initial consultation has been completed, we will refine this panel to your needs and may also ask for additional testing from your GP.

We wish to assist you in uncovering the underlying biochemical contributing causes of your mental health symptoms as well as how trauma has affected you physically, and this will give us answers.

  • Bioscreen microbiome panel

Bioscreen Medical FMA. This test determines the levels of gut bacteria in the digestive system. Gut bacteria determine brain chemicals (neurotransmitters), happiness, energy, stress levels, nutritional levels, interactivity, sensory input, gut function, immunity and allergies.

The FMA test analyses the gut microbiota using classical culture and the most up-to-date identification techniques. Many scientists (including clinical microbiologists) regard this technique as the ‘Gold Standard’ for microbiological testing (The Rebirth of Culture in Microbiology through the Example of Culturomics To Study Human Gut Microbiota, J-C. Lagier et al., Clinical Microbiology Reviews, 2015, 28,1, 237-264).

The FMA Test is a comprehensive analysis of the gut microbiota from the large intestinal tract. Bioscreen cultures, quantifies and identifies these bacteria. These organisms include organisms that can grow in the presence of air (aerobic bacteria, yeasts) such as coliforms, Enterococcus, and Streptococcus spp and organisms that cannot grow in the presence of air (anaerobic bacteria) such as Bacteroides, Eubacterium, Bifidobacterium, Lactobacillus and Clostridium. We also culture yeasts such as Candida, Saccharomyces spp.

Bioscreen identifies all gut bacteria by MALDI-TOF mass spectrometry technique. MALDI-TOF is an acronym for Matrix Assisted Laser Desorption Ionization Time of Flight spectrometry. The MALDI-TOF is a state of the art analyser where bacteria are vaporized using a laser beam to produce a cloud of peptide molecules (small protein molecules). These peptides are separated in the spectrometer to produce a spectrum of different size peptides or a ‘Peptide Fingerprint’. The peptide fingerprint that is characteristic of each bacterium. MALDI-TOF spectrometry is reliable and can identify many thousands of distinct bacterial species.Routine stool examination as performed in other and mainstream pathology laboratories is focussed on isolating enteric pathogens. Bioscreen testing, by way of contrast, provide quantitative data on commensal gut bacteria, but not enteric pathogens like food poisoning organism.

A comprehensive report is produced detailing the microbiology of your faecal sample. The report will allow your practitioner to give advice on how a change of the gut microbiota, if identified by the test, may be corrected. This advice can include modification to your diet, a diet specifically targeted for your gut microbiota, the use of probiotics and in some cases specialised antimicrobial herbs or antibiotic therapy. Bioscreen does not give therapeutic advice directly to patients. Scientists from Bioscreen are always available to give advice to the healthcare practitioner who, for medicolegal requirements, must always take responsibility for patient's care.

My DNA multiple-category Medication test ( swab)

The pharmacogenomic profile identifies which medications are most suited to an individual. Common conditions include Depression, Schizophrenia, Bipolar disorder, epilepsy, seizures, ADHD, anxiety, and other neurological disorders. This test lets us know how the body metabolises medications and can come in very useful.

Pharmacogenomics (PGx) is the study of how genetic variations influence the way an individual responds to medications. PGx enables doctors to test for specific genetic changes to predict whether a patient may have a normal response, a poor response or a higher risk of side effects, before prescribing a specific medication. The Potential Benefits of The Test;

  • Improve the patient’s quality of life.
  • Reduce the number of adverse drug reactions.
  • Avoid ineffective medications.
  • Save the patient time and money on trial
  • and error medicines.
  • Help patients reach treatment goals sooner.

Supplementation

Already underway and budgeted

Once pathology is completed and sent to the labs, we can start you on Initial supplementation to help you to feel better and assist in stabilisation. At the same time, we wait for pathology results to come back, and we can then refine what you are taking based on your results. This will continue through the residential and post-programme support phase and will be monitored weekly. This can take the form of prebiotics, probiotics, vitamins and minerals in a form the body absorbs well, amino acids, herbs and IV nutrition. This goes hand in hand with dietary support.

Psychotherapy

(trauma-informed)

We move into intensive face-to-face blocks of therapy to continue the work and training required at a deeper level for the beginning of remission of the presenting conditions. This is a safe treatment space in your own apartment with your trauma-informed specialist who will teach and equip you with a mix of highly effective self-regulation techniques and skills

Residential: Four x two-hour private sessions over two days.

Psychotherapy

- Family and Systemic Constellation Psychotherapy - -

(Trauma-Informed)

Family Constellations work is widely recognised as an effective approach for addressing systemic concerns. Such concerns commonly encompass family-of-origin matters, challenges in parent-child relationships, and difficulties within intimate relationships. This therapeutic method holds considerable potential for individuals who:

  • Have experienced significant trauma or loss and seek healing and recovery.
  • Seek to address and break harmful relationship patterns.
  • Are actively attempting to resolve entanglements within their family dynamics.
  • Desire to overcome inner turmoil and find inner peace.
  • Wish to calm their central nervous system

In 1:1 sessions, symptoms or issues are looked at using symbols on a table to represent the different parts of the family or social situations contributing to the development of symptoms. The symbols are used to recreate the parts of the system, which include:

-Self now

-Self at the time of trauma or loss

-Resources: strategies that are supportive or strengthening, e.g. nature, pets, safe memories, partners, friends

-Defences/survival strategies that were acquired, e.g. hyperarousal, hypervigilance, fight/flight/freeze/fawn, social withdrawal/hiding, hopelessness

-Relevant family members, including parents, ancestors

-Social factors where bullies/perpetrators can be faced to reclaim what was taken, e.g. trust, innocence, inner strength, one’s voice, sense of agency

-Intentions/desired outcomes such as belonging, feeling calm, re-energised, healthy boundaries, emotional regulation, inner balance/feeling centred or grounded

Each part of the system holds systemic intelligence/information that can be accessed through physically touching each of the symbols/representations, bringing unseen and unknown material into consciousness, and revealing impulses/movements where the system wants to re-organise itself to fulfil the client’s intentions/wishes.

At the end of each session, the client has a felt sense in their body of how it is for the issue to be resolved. New resources are also internalised, such as somatic resources (feeling lighter, less burdened, easier breathing, at ease in their nervous system, clear-headed, and less pain). A new picture of the system is also internalised as an imaged memory that can be taken in to anchor the somatic resources. Healing sentences are spoken to heal the parts of the system where order was lost that can now be reclaimed.

It is a powerful therapy process that can be used for different issues one has faced throughout their lifetime and even through previous generations.

Appointments three , two hour sessions in residential

Psychotherapy

- Clayfield Assisted -

(Trauma-Informed)

Also known as Clay Field Therapy uses a wooden box (to contain the clay) on a table filled with 15kg of Clay from the Earth. It comes with a warm bowl of water and sponge, a discard bucket on the floor and is often done with eyes closed for Adults. This approach has been a mainstream, very efficient therapy in Europe for 30 years.

The box and Clay become a 3D blank screen for the nervous system and psyche to project into, to tell the story directly from the nervous system’s perspective through hand movements e.g. to push away to create safety, boundaries, distance or protection. The hands may wish to reach to find resources such as a solid, reliable hold that is stabilising. Create an underground bunker that is protective and strengthening. They may also wish to access the warm water to learn how to regulate the flow of water in and out of the box to achieve emotional regulation or use the warm water to create conditions of soothing or comfort, ease, manageability, softness or warmth so the nervous system can down-regulate.

Strong emotions can be processed and released e.g. trapped fight, flight responses, suppress anger or rage, or protest can be released from the body with hands squeezing the clay, destroying how it has been in order to make room for new Life. The water can also be used for cleansing where there have been boundary violations such as abuse, but also for purifying or blessing parts of self-represented in the clay that need to have vitality restored to them.

Clay Field Therapy also provides a powerful, visceral experience as it is a touch therapy using haptic perception through sensing the material with receptors in the skin of the hands that map to all areas of the Brain’s Cortex. The hand movements are driven by an impulse for the client to find balance, wholeness, healing, and transformation that is mainly at an unconscious level. There may be little or no talking as inner images, movies, sensations, or emotions come up. They can all literally be handled with an active response to the Clay. This therapeutic tool for trauma healing is available for both adults and children. The emphasis is on the sense of touch and the movement of the hands during the clay therapy sessions, and it does not rely on an end product being made.

The tactile experience of working with clay provides a unique and intense sensory experience for the client. By engaging with the clay through touch, the therapist can address various emotional and psychological issues related to trauma. These may include early attachment problems, difficulties in development, and past traumatic events.

One of the advantages of using Clay Field Therapy is that it primarily relies on nonverbal communication. This means that clients can express themselves and explore their emotions without necessarily having to use words. The therapist can guide the client through the process using a body-focused approach, paying attention to the sensations and movements experienced while working with the clay.

The kinaesthetic motor action of the hands refers to the physical movement and manipulation of the clay. This hands-on approach allows clients to actively engage in the therapeutic process. As they mould and shape the clay, they can explore and express their emotions, experiences, and inner world.

Additionally, the sensory perception involved in working with clay stimulates various senses, such as touch, sight, and proprioception (the awareness of one's body in space). This multi-sensory experience can have a profound impact on the client's healing journey.

Overall, the combination of the tactile experience, kinaesthetic motor action of the hands, and sensory perception during clay therapy sessions can lead to a deep sense of resolution. This resolution can bring about lasting therapeutic benefits, helping individuals heal from trauma.

Appointments: three, two-hour private sessions on residential.

Psychotherapy

(Trauma Informed and trained Art-Assisted & Self Development)

Closely linked and integrated with equine-assisted psychotherapy is art-assisted psychotherapy.

Sensorimotor Art Therapy has the potential to initiate a process that unfolds like an inner healing journey. Guided Drawing represents the key approach of Sensorimotor Art Therapy. It is a body-focused drawing approach providing an archetypal structure that applies the philosophy of Jungian Depth Psychology to universal, formal elements, like a line or a circle or a square. The aim, however, is not so much to teach one particular approach but to understand art therapy as a visual principle, an open concept that can be translated into a wide range of different contexts.

While a lot of the focus of the training program will be Guided Drawing, other art therapy approaches will also be taken into account. The aim is to teach a variety of different tools suitable for a variety of different client groups and inner needs.

In recent years “sensorimotor” has emerged as a term to describe body focused psychotherapies that use a bottom-up approach. Instead of a cognitive top-down strategy, sensorimotor art therapy encourages the awareness of innate motor impulses in the muscles and viscera, also as heart-rate and breath. The expression of these motor impulses followed by their perception through the senses, allows the development of new neurological pathways that can repair traumatic memories; such an approach is capable of restoring wholeness and wellbeing.

Both Guided Drawing and Work at the Clay Field are sensorimotor, body-focused, trauma-informed art therapy approaches. They are not necessarily concerned with an image-making process, but support the awareness of body memories. While these memories are always biographical, the therapy itself is not symptom-oriented. Not the specific problem or crisis becomes the focal point, but the option to new answers and solutions as they are embedded in the body's felt sense. Such sensorimotor achievements are remembered similar to learning how to swim or ride a bike. They are lasting achievements that can transform even early infant developmental set-backs; they assist in finding an active response to traumatic experiences. They allow us to rewrite our biography towards a more authentic, alive sense of self.

Appointments: Six x two-hour private sessions

Psychotherapy

(trauma-informed and trained equine-assisted therapy)

Together with non-talk therapy, we make equine-assisted therapy (EAGALA) a component of your program. Equine therapy is hard to explain as it is different for everyone. Still, essentially it is using horses who pick up body language and micro-expressions far more efficiently than humans as a form of a feedback loop.

Why horses? There are several reasons we choose to use horses in this work, but primarily it is due to their nature and instincts as social and prey animals. They have an extraordinary ability to receive and express emotional context without the spoken word or judgment. They are experts in nonverbal communication and live in the present and are aware of subtle changes in us and pick up on the messages we are sending, which we are not always conscious we are doing. With this, our stories can unfold, and they may either show us or provide opportunities to change what those stories look like.

Horses do not know our past, education, gender, race or other labels we may apply to ourselves and each other. They are in the moment and can be a part of this relationship without the biases we humans put on each other. This provides even more value in the insight they can provide us about ourselves.

EAGALA is used worldwide for veterans and first responders to deal with depression, anxiety and mental health resolution.

The highly trained EAGALA therapists in your programme (2 at all times throughout therapy) will assist you through a memorable experience that:

  • Reduce anxiety, stress and depression
  • Process traumatic events
  • Work on self-awareness and development
  • Works on the sense of self.
  • Express externally what is happening internally.

Appointments: four x two hour-hour private sessions with two therapists

NeuroAcoustic Sound Therapy

Sound Therapy is individualised and regulates your Autonomic Nervous System (ANS) to achieve a Zero-Stress State (Homeostasis). That is, a state of equilibrium and balance between the stress handling and relaxation components of the ANS, thus allowing healing to occur. This feels great!

In each phase of the Sound Therapy process, with the use of some amazing technology, you hear and feel your individualised vibration and frequency, with each of the six phases working to balance and re-train the nervous system. We will monitor you during each weekly session and provide visual feedback on the state of your ANS so progress can be shared

Appointments: Initial consultation plus five follow-up sessions with a shared unique sound file made for you to be used anytime. This is especially useful to use before bed.

Red Light Therapy

(Photobiomodulation therapy)

Much like the sun gives us Vitamin D, Photobiomodulation (light) therapy is a fast-growing technology used to treat a multitude of conditions that require stimulation of healing, relief of pain and inflammation, and restoration of function.

Red and near-infrared light therapy has been clinically proven to:

  • Restore mitochondrial oxidation and energy production
  • Assists with sleep function
  • Speed muscle recovery and enhance peak performance
  • Heal acne and blemishes
  • Fade scars and stretch marks
  • Faster wound healing
  • Reduce joint inflammation
  • Aid recovery from Hypothyroidism
  • Enhance collagen synthesis and repair

Red and infrared light help our mitochondria ( the energy centre of the cell) function better. Better functioning mitochondria mean more energy and less risk for disease.

The skin is the organ that is naturally exposed to light more than any other organ. Red and near Infrared are the only wavelengths that penetrate through the skin. Mitochondrial chromophores in skin cells absorb the photons. Consequently, electron transport, adenosine triphosphate (ATP) nitric oxide release, blood flow, reactive oxygen species increase and diverse signalling pathways activate. Stem cells can also be activated, allowing increased tissue repair and healing, reducing inflammation and more energy to the brain and body. Helping you to feel better but also keep the gains you have achieved through other therapy.

Appointments: We give you access to a cranial red light helmet to use daily, as well as access to a full-body red light bed when doing sound therapy

Stellate Ganglion Blockade (SGB) with PBM

SGB using PBM has been recently identified as producing several positive effects, including improved blood flow by dilating vessels and reduced pain. The mechanism of action is the same as standard SGB using local anaesthetic in patients with neuropathic pain by directly blocking the afferent nociceptive signals through sympathetic pathways. SGB has also been used in clients with PTSD and is thought to work by inhibiting connections between the peripheral sympathetic nervous system and regions of the cerebral cortex, which are abnormally activated in PTSD. There is extensive research on this being effective in using an invasive and expensive anesthetic. We find this the be a much gentler alternative.

Japanese Acupuncture

Japanese acupuncture is a branch of traditional acupuncture that originated in Japan. It shares many similarities with traditional Chinese acupuncture but also has some distinct characteristics. Japanese acupuncture focuses on gentle needling techniques, using thinner needles and shallower insertions compared to Chinese acupuncture. It emphasises palpation and assessment of the body's energetic imbalances before needling. This style also places a strong emphasis on the practitioner's sensitivity and intention during the treatment.

Japanese acupuncture has been recognised for its benefits in promoting mental health. The treatment restores balance to the body's energy, or Qi, which can positively influence the mind and emotions. By stimulating specific acupuncture points, Japanese acupuncture aims to regulate the flow of Qi and blood throughout the body, reducing stress, anxiety, and depression. Moreover, the gentle needling techniques employed in Japanese acupuncture provide a calming and relaxing experience, allowing the recipient to enter a state of deep relaxation and emotional balance. Additionally, the holistic approach of Japanese acupuncture acknowledges the interconnectedness of the body and mind, aiming to address underlying imbalances that may contribute to mental health issues. Overall, Japanese acupuncture offers a holistic and gentle approach to improving mental well-being by harmonizing the body's energy flow and promoting a sense of calm and balance.

Infrared saunas have been recognised for their numerous health benefits, particularly when it comes to mental well-being. Firstly, regular use of infrared saunas can aid in stress reduction and relaxation. The gentle heat emitted by the infrared waves penetrates deep into the body, promoting muscle relaxation and releasing tension. This helps to alleviate both physical and mental stress, leading to a sense of calm and relaxation. Additionally, the heat stimulates the release of endorphins, which are natural mood-enhancing chemicals in the brain, promoting a positive mood and reducing symptoms of anxiety and depression. The overall experience of warmth and tranquillity in an infrared sauna can have a soothing effect on the mind, providing a much-needed break from the pressures of daily life.

Three sessions combined with a magnesium bath soak ( magnesium is a natural muscle relaxer and can also add to increased mood).
